Who Case Managers Are
Case managers are trained professionals in the field of social services. They can meet with you in the privacy of your home to discuss your personal situation. Your case manager will develop a "care plan" with you and help connect you with services in your community. They can also help you obtain homemaker services if you require some assistance at home.
How Case Managers HelpAging individuals living at home sometimes need assistance to remain independent. The elderly and those who care for them can sometimes be overwhelmed by the strain of meeting these needs. Case managers are able to help you meet these challenges by providing professional guidance.
What Case Managers Do
Visit you in your home and assess your situation. Help you design a "care plan" and link you with services. Stay in touch to make sure you are receiving the care you need.
There is never a fee for any OFA service. Voluntary contributions are accepted and used to expand the program and will be greatly appreciated. Programs are funded by the City of Yonkers and federal and state grants through the NYS OFA and Westchester County Dept. of Senior Programs and Services.
