Birth and death certificates are recorded and filed in the City Clerk’s Office for those individuals who are born, or die, in the city. Birth and death records are confidential records under New York State law. Genealogical searches and fees are covered separately.
To obtain a Death Certificate in Person or by Mail
Please follow the same guidelines as for obtaining a birth certificate, include the date of death and the relationship to the deceased. Mother’s maiden name is not necessary when requesting a copy of a death certificate. The fee is $10.00 per copy, payable by cash, personal check or money order. Again, please include a self-addressed, stamped envelope.
Please note when obtaining a parent's death certificate, you must also produce your birth certificate showing your parent's name.
