Birth and death certificates are recorded and filed in the City Clerk’s Office for those individuals who are born, or die, in the city. Birth and death records are confidential records under New York State law. Genealogical searches and fees are covered separately.
To obtain a Birth Certificate in Person or by Mail:
The City Clerk’s Office is located in Room 107 at City Hall, 40 South Broadway, Yonkers, New York 10701 (corner of Nepperhan Avenue and South Broadway) telephone 377-6020. The office is open from 9:00 a.m. until 4:00 p.m. Monday through Friday. The applicant must be at least 18 years old and must have a photo identification. Birth certificates can only be obtained by the individual him/herself, or a parent. The applicant will be required to complete a form containing the following information: name of person for whom the certificate is being requested; date of birth; parents names, including mother’s maiden name, relationship to the applicant and the reason the certificate is needed.
Please include this information if applying by mail, including photocopy of photo identification, and a self-addressed, stamped envelope. Copies are $10.00 each, payable by cash, personal check or money order.
